Instructions:

After you click the link above you will come to a login page with another link at the top that will bring you to our new hazardous waste disposal request form. Requests can also be made through the EH&S Assistant login on this page but to access this requires prior authorization from the Environmental Health and Safety (EHS) office.

When filling out the form, please use your laboratory room number and building (or anything else you will remember) in the "ID Number" section. By doing so, when you visit the site again using the same ID, most information will autofill when you press the autofill button.
