Instructions:

After you click the link above you will come to a login page with another link at the top that will bring you to our new hazardous waste disposal request form. Requests can also be made through the EH&S Assistant login on this page but to access this requires prior authorization from the Environmental Health and Safety (EHS) office.

When filling out the form use an "ID Number" that you will readily remember (your NU ID, lab location, pet name, etc.). By doing so, when you visit the site again using the same ID, most information will autofill when you press the autofill button.

Pick-ups of hazardous waste from laboratories are every Friday. Please submit requests by the end of the day on Thursday to ensure your laboratory is on the list.
